What you will be doing:
- Provide in-home, in-office, and/or community assessments of the emotional, social, and physical needs of older adults reporting any form of abuse or mistreatment.
- Develop and implement a comprehensive assessment, service plan, and safety plan for the provision of a full range of social services to address the abuse experienced by the older adult and, if appropriate, the alleged abuser.
- Complete required web-based case documentation in accordance with funder requirements.
- Offer supportive counseling and guidance in the resolution of problems faced by the older adult and/or the alleged abuser, family member, or caregiver.
- Refer to Legal Services for Elder Justice and/or other legal organizations.
- Provide appropriate resources to older adults and collaterals regarding coping mechanisms and ways to preserve individual functioning and autonomy within the context of the abuse experienced by the older adult.
- Screen for eligibility in securing and maintaining benefits and entitlements such as transportation, Home Care, Meals on Wheels, SNAP, Public Assistance/Case Assistance, Medicare, Medicaid, emergency financial relief, etc.
- Make referrals to community-based organizations.
- Identify high-risk, crisis, and/or emergency cases.
- Conduct group counseling to strengthen and empower older adults and build community among their peers.
- Participate in community presentations, education, and outreach activities.
- Engage in internal and external training, workshops, and conferences.
- Attend weekly individual and group supervision and all other mandatory meetings as assigned.
- Perform other duties as assigned.
Experience you will need:
- Graduate of an accredited college or university with a Master's of Social Work (MSW) or equivalent Master's-level degree in Gerontology, Psychology, Human Services, or other Social Services-related field.
- Exemplary written and verbal communication skills.
- Experience in individual counseling, utilizing a variety of treatment modalities, including, but not limited to, Motivational Interviewing.
- Proficient in using computers and able to learn and utilize program-specific software for daily data entry.
- Ability to travel to clients' homes using mass transit and to negotiate any barriers in clients' residences.
- Bilingual preferred (Spanish, French-Creole, Mandarin, Hebrew, etc.).
- An understanding and appreciation of the roles of race, religion, ethnicity, gender, sexual preference, and individual values as they relate to serving vulnerable and at-risk older adults.
Salary: $50,000 - $65,000/year
